Forms

For your convenience, forms can be filled out electronically. (Note: Signatures can be taken care of when you visit the office.) To do this, there are two methods, whichever you prefer:
1) Click and open the form and fill it out. Then click the print button in the top left and bring it to the office.
2) To email it instead, you can right click the link for the form you need, go to "Save Target As.." or "Save Link As..." and save it to your desktop. Then open it up, fill it out, click save and then attach it as an email to staff@drkevinemery.com. (Newer versions of Adobe Reader let you click an envelope in the top left to quickly attach to email that way instead of having to save it to your desktop.)
